  • CE Youth Agreement

    HEALTH/MEDICATIONS: Please note that Community Education does not have access to the school health offices, where medications are typically stored, outside of the regular school day. If your child has medication or health needs that may be of concern during participation in this program, you must contact Community Education at 952.226.0080 ASAP to discuss these needs, and required paperwork, ASAP before the program begins. CANCEL/TRANSFER: We MUST receive request no later than 4:00pm, 5 business days prior to the first day of class. ABSENCE: We WILL keep your student for class if we are not notified at #952-226-0080 that they should not attend. LATE PICK-UP: Students MUST be picked up by an authorized pick-up person in the classroom by the end of class, late pick-ups WILL be charged a fee. POLICIES, PROCEDURES & BEHAVIOR: Community Education (CE) is a program of ISD #719 and follows PLSAS policies and procedures. Registration implies agreement for students and families to adhere to all of the policies and procedures of both CE and ISD#719. Failure to comply with policies, procedures, and behavior expectations WILL result in immediately dismissal from the class without a refund. EXCHANGE OF INFORMATION: I give my consent to an exchange of my students information between CE and other PLSAS staff whenever it may be deemed as beneficial to my student. LIABILITY WAIVER: I give PLSAS & CE staff and instructors permission to take whatever emergency measures deemed necessary for the care and protection of my student during their participation in a CE class. In case of extreme emergency, I understand my student will be transported to the nearest hospital by local emergency unit for treatment as deemed necessary. Any expenses incurred are the responsibility of the parent/guardian. I understand that registration for this class waives all rights and claims for any and all injuries from whatever cause suffered by participation in class-related activities. PUBLICITY PERMISSION: If I do not wish to have my student included in pictures/publicity, I understand that I must contact Kristi Mussman, PLSAS Director of Communications, to obtain the photo opt-out form. In addition, I understand that it is my responsibility to complete and return the form to Kristi Mussman. PERSONAL ITEMS: I understand that CE is not responsible for lost, stolen or damaged items.
